 入會方法

 

 請填妥「入會申請書」並連同
• 2 張相片
• 視光師註冊証明書副本1份
• 視光師週年執業証明書副本1份
• 劃線支票港幣$1,500 元正, 支票抬頭「香港光學會有限公司」

$780 元正為該年度會費「有效期至12月31日」、視光師的入會基金是$1,500元正，共合共$2,280元正。

寄回本會會址：
香港光學會中環 己連拿利一號 華昌大廈 廿二樓 2102 室
