 1 合資格報名參加的視光師
-只有在委員會註冊名單上的視光師可以參加，正申請覆牌或已被除牌的不可以參加
-義工組會先個別了解視光師實際欠多少學分和不能獲得足夠學分的原因，然後轉交教育小組審批。

*視光師可因應個人需要決定是否參與是次特別加開課程 或 申請成為其中一個預獲核准持續專業發展課程的籌辦機構的會員。
特別加開課程以實體形式進行並只舉辦這一次，其他週期不會再舉辦。

1. 本課程只接受支票付款, 收費以每日計算。( 例:參加者報讀了三天的課程便需分別填寫 3 張支票。)

4. 課程報名一經確定後不得更改或取消，所有已繳付之費用不設退款，課程名額亦不得轉讓。
