This message serves to remind you to keep the Health Care Voucher Division informed of any changes of your particulars recorded in eHealth System (Subsidies), including your personal particulars, particulars of the Medical Organization, the practice details and bank details. Should you have any amendments of particulars or wish to withdraw from the Health Care Voucher Scheme, please complete the “Request to Change Particulars” form and send to the Health Care Voucher Division as soon as possible for updating the record.
